If you were born outside Canada to a Canadian parent on/or after February 15, 1977, you must send following documents and follow procedure mentoned below:
  • a birth certificate which lists your parents (issued by government authorities in the country where you were born)
  • proof that one or both of your parents was a Canadian citizen when you were born (for example, citizenship certificate or Canadian birth certificate)
  • two (2) pieces of personal identification, such as a driver’s licence and a health insurance card
  • 2 Photographs
  • If the application is for a person under 14 years of age, it must be signed by a parent or guardian in the space provided. Indicate the place and date.
  • submit your application to the Canadian embassy, high commission or consulate closest to your home along with above mentioned documents.
  • If one of the parent is in Canada after approx. two moths of submission of citizenship application sign ppt 028 form and go to Canadian passport office in Canada and have it faxed through passport office to Canadian high commision overseas.
